Acupuncture Clinic Amsterdam strives to provide the best care through personally designed health treatments. In order to gain the most benefits of these treatments, the client must be a participant and have a direct, complete and personal relationship with their health care provider.

Acupuncture Clinic Amsterdam respects the privacy of our clients. This privacy policy is intended to explain and describe the information we collect, how that information may be used, with whom it may be shared, and your choices about such uses and disclosures. We strongly encourage you to read and understand this privacy policy fully. By becoming a client at Acupuncture Clinic Amsterdam you acknowledge and agree that you have read and fully accept this Privacy Policy.

Acupuncture Clinic Amsterdam acts in accordance with legal regulations. The rights and obligations of the client and the care provider are recorded – amongst other things – in the Medical Treatment Contracts Act (WGBO) and the Quality, Complaints, and Disputes Act in Healthcare (Wkkgz).

Acupuncture Clinic Amsterdam keeps a medical record containing personal data, medical history, family history as well as details about the medical examinations and treatments performed. It also contains details that are necessary to your treatment and those we, where applicable and only with your explicit consent, have obtained from another caregiver, such as your general practitioner. The clients have the right to inspect their own file. The legal retention period of your medical record is twenty years.

In accordance with the General Data Protection Regulation (GDPR), Amsterdam Clinic Amsterdam honours and protects your privacy. At the intake, we will ask your permission to record and process your details and work with a treatment agreement. By making an appointment and undergoing treatment, you agree to both the declaration of consent for data processing and the treatment agreement. Acupuncture Clinic Amsterdam  only provides your details to third parties, when this is necessary for the execution of our treatment agreement or to comply with any legal obligation.

During the intake and before the treatment you should inform us of any medication use, pregnancy, or treatment by a doctor, therapist, specialist, or other practitioner. It may be necessary to make adjustments to the treatment based on this information. Acupuncture Clinic Amsterdam is not responsible for damage and/or injury resulting from the concealment of medical information such as physical and/or mental conditions, pregnancy, injuries, and medication use.

Appointments and Cancellation Policy

Appointments can be change or cancelled 24 hours in advance, without charge. In case of cancelation within 24 hours, the full amount of the missed appointment will be charged.

Payments are made directly after the treatment. If required, an invoice can be sent by email after payment.

Acupuncture Clinic Amsterdam is not responsible for damage, loss, or theft of personal belongings.

Complaints Procedure

We strive to offer you treatments of the highest quality and care. We would like to draw your attention to the following conditions and applicable complaints procedure (Klachtenprocedure NVA conform de Wkkgz). In case you are dissatisfied with any aspect of our services – and the issue cannot be resolved together – you can seek support or submit a complaint to the professional association NVA (link), of which we are a member. All information is confidential. In case of any disputes, you can contact Stichting Zorggeschil.

Declaration of consent for data processing

Under the General Data Protection Regulation (GDPR) as of May 25, 2018, Acupuncture Clinic Amsterdam is required to inform its clients of what data is being recorded and who has access to this data.

To offer the best treatment possible, keeping a medical record is necessary. This is also a legal obligation imposed by the Medical Treatment Contracts Act (WGBO). The medical record contains notes about the client´s health condition, underlying patterns as well as the performed examinations and treatments. This data is recorded, carefully stored, and kept for 20 years as prescribed by the WGBO.

We honour your privacy and ensure professional secrecy. During the intake your permission to record and process your data will be asked and recorded in your medical record. The treating therapist is the only one who has access to the data in the medical record and has a legal duty of confidentiality. The data that will be recorded includes the following:

General data:

• Name, address, postal code, place of residence

• Date of birth

• Gender

• Phone number

• email address

• General practitioner

• Citizen service number (BSN)

Anamnesis data:

To get a complete and holistic picture of the client´s health, we have an elaborate anamnesis during the intake. The following data will be requested, recorded, and kept:

• main complaint as well as additional complaints

• chronic conditions / surgeries / accidents / medical conditions from the past / possible pregnancies / childbirths

• life events / traumas

• medication / supplements / drugs

• family / home situation / work / sports / hobby

• various questions to gain a complete picture of one’s state of health and make a correct diagnosis according to Traditional Chinese Medicine

Access and sharing of data

As a client you have the right to see, change or delete your personal details. You also have the right to withdraw your consent for processing your personal details or to object the processing of your personal data.

Upon request, the client has the right to gain access to the medical record and if needed, a copy may be provided. In case of removal or destruction of your personal data it is necessary to file a request in writing.

The medical record contains data that is necessary for your treatment and that the therapist, with your explicit permission, can obtain from another healthcare provider; for example, your general practitioner or a specialist. Upon request and only in consultation, your data will be shared with other parties. Written permission from the relevant client is required. Only when legally necessary, your data may be provided to third parties without your consent.
